Public Health Wales is working on a plan to make healthcare fairer for everyone. They want to hear from you about what good and fair healthcare means to you.

What You'll Do
There are six questions about your experiences with primary care services (like doctors, nurses, pharmacists, and dentists). You can answer these questions in different ways: discussing in a group where one person could complete on behalf of the whole group, or writing your answers individually —whatever works best for you.

By sharing your experiences, you'll help make healthcare services fairer for everyone. Your voice is important, and Public Health Wales (PHW) appreciate your time and insights.

Privacy 
Your answers will be kept private. PHW)  won't use any details that can identify you in their reports. The information you share will help PHW plan to reduce health inequalities. You don't have to take part, and you can stop at any time without giving a reason. Please do not include any personal information in your answers, any identifiable information shared will be excluded from analysis
